Classroom Volunteer
To volunteer in the classroom, you will need to submit the Volunteer Application to the front office every year.
In addition, you need proof of TB clearance within the last 4 years. This needs to be renewed every 4 years. If you are eligible for the questionnaire, there is a form below you can take to the doctor for signature.
TB clearance lasts for 4 years.
Field Trip Volunteer
All field trip chaperones are required to have LiveScan fingerprint clearance with DOJ & FBI to attend a field trip in ADDITION to the Volunteer Application and TB clearance on file.

Livescan fingerprinting lasts your entire time at MATES. No renewal needed.
Parent Drivers for Field Trip
We use buses for transportation to field trips. On occasion we may need parent drivers. Once confirming with the teacher you will be a driver, you need the following to drive:
- Volunteer Application
- TB Clearance
- Livescan Fingerprinting
- Drivers Packet including proof of insurance
