MATES Cafeteria Order Form

All meals are provided at NO COST and includes the option of 1% White milk, Fat Free Chocolate milk, and the vendor’s choice of fruit and vegetable daily.
 
Breakfast will be distributed at morning recess. You will use the same order form, just continue through the lunch options and the breakfast options start after the lunches.

Please Note Menu Changes/Substitutions May Occur: 

The Child Nutrition Department is experiencing some severe food delivery shortages due to the supply chain issues you may have heard of in the news.  We apologize for any menu changes or substitutions that may occur and will do our best to keep them to an absolute minimum.  Thank you for your understanding.
